Certificate Requests

Certificates are intended to commend individuals, community organizations, or businesses for their exceptional contributions to the people of the City of Los Angeles.

General Guidelines:

Requests must be submitted four (4) weeks before the date needed.
All requests will be reviewed and responded to in a timely fashion. Given the high volume of requests, our office may not be able to accommodate your request, even if the criteria outlined above are met.
For requests with completion dates within less than 4 weeks, contact CD6.certificates@lacity.org
Certificates must be picked up at City Hall, Van Nuys, or Sun Valley Offices ○ Note: Parking accommodation is not available for any location.
Our office will communicate with the requester once the certificate request has been approved. We will coordinate the completion and pick-up date.
The requester is responsible for entering accurate information and reviewing the form before submitting the certificate request.
No prior certificates have been provided to the honoree or organization for the same recognition.
If you would like multiple certificates with the same recognition language, please submit them in one request with an attached list of names
For multiple honorees, with different certificate types(s) of recognition, submit one form for each honoree
Certificates for self may not be requested.

Certificate Type Options:

Recognition – Certificate recognizing occasions of exceptional community contributions.
Congratulations – Certificate recognizing occasions of exceptional significance: Grand Opening for Small Businesses, Ribbon Cutting Events.
Anniversary – Certificate recognizing milestone anniversaries, and business/organization service years.
Retirement – Certificate recognizing a retirement from the City of Los Angeles, LAUSD employee, other government entity, or a nonprofit organization.
Wedding – Certificate recognizing weddings.
Commendation
– Certificate recognizing outstanding service of excellence.
Birthdays – Certificate recognizing birthday milestone.
